Accessing your personal information

The Data Protection Act 1998 allows individuals to access their own information, while at the same time protecting individuals’ personal information from inappropriate use or disclosure.

The Trust has produced a leaflet for its service users and patients to inform them about how it processes their personal information.

If the Trust holds personal information about you, you can request access to your information by speaking, in the first instance, to the member of staff providing your care and treatment. If you wish to be provided with copies of your information, you will need to put your request in writing, and you may be required to pay an administration charge.

The following form may be used to apply for copies of your information.
