Community/District Nursing

   

The community nursing service provides nursing care in the community, both in the patient’s own home and local health facilities.

District nurses are skilled professionals who provide a wide range of person centred nursing care and support to patients based on the assessed needs of the individual. A named District Nurse is aligned to each GP practice and works in partnership with other professionals within the Integrated Primary Care Team including social workers and occupational therapists. 

They provide high quality care to both acutely and chronically ill patients, and are frequently the identified Key Worker responsible for co-ordinating care for people with Palliative and End of Life care needs.

The community nurse supports patients/families cope with ill health and disability thus allowing them to reach the best possible quality of life, whilst maintaining dignity and respect.

Referrals to Community nursing
Access to the Community Nursing Service can be made by yourself or via a family member, GP or other health professional.
Monday – Friday, 9.00am - 5.00pm

All referrals are handled centrally by a Call Referral Management System (CRMS)
