Permanent Placement Team

The Permanent Placement Team is responsible for reviewing the care of people who are placed in permanent residential and nursing care homes in the statutory, voluntary and independent sector.

Part of the Permanent Placement Team work is to ensure that when abuse or neglect is suspected, residents are safeguarded and the circumstances investigated.  To support this work the team now have  Senior Social Work Practitioners, a social worker and Nurse Specialists for Safeguarding.

It consists of two teams of nurses and social workers which operate trustwide. The Northern Team, based in Route House, Ballymoney, covers Ballymena, Larne and the Causeway localities and the Southern Team, based in Dublin Road, Antrim, covers Newtownabbey, Carrickfergus, Whitehead, Antrim, Magherafelt and Cookstown.

They work across the mental health, physical disability, learning disability and older peoples programmes of care. Nursing staff in the team also complete the assessment for eligibility for the Health and Personal Social Services (HPSS) Payments for nursing care in nursing homes.

Referrals to the team are accepted when the person has been resident in a home for three months and the placement is settled.

Hours of service

Services are available throughout the Trust from 9 am – 5 pm, Monday – Friday, excluding bank and public holidays.
