Carer Assessment

A Carer Assessment can be carried out which gives carers an opportunity to tell us about the care they provide and how it affects their life. Carers Assessment Leaflet

How we keep you informed

As part of the Northern Trust’s commitment to support family carers we have a mailing list/database of family who support or care for a loved one. We call this database the Carers Register. It is simply a contact list that we use to keep you up to date with the support programme and what’s available across the Trust area. If you are a family member who is looking after or supporting a loved one and you would like to be kept informed with what’s available please complete a carer connect card. Carer Connect Card

When caring ends or changes

Looking after someone may be a large part of your life but it is inevitable that your caring role will change or end over time.  Whatever your situation you can find help and support.  Carers NI have developed a resource which you may find useful. When Caring ends or changes

Carers Digital

The Northern Trust has teamed up with Carers NI and Carers UK to offer further support to carers across this area via digital resources tailored to their needs.

Carers can now have 24/7 access to a range of resources hosted on an online platform including, eLearning, “Jointly” a care co-ordination app, a self advocacy guide as well as many more links to local information and support for carers specific to the Northern Trust area. The platform can be accessed using smartphone, tablet or on a web browser. The aim of the resources is to help carers manage tasks and care.
