The Dementia Home Support Team provide behavioural assessment of individuals with dementia.

Referral information

The single point of referral for behavioural assessment is to Community Mental Health Teams for Older People.

Hours of service

The service operates from 9.00am – 5.00pm, Monday to Friday, except public holidays.

The Dementia Home Support Team are a Trust wide service and staff are based in Antrim, Ballyclare, Coleraine and Magherafelt.
