My Care – your patient portal

My Care is the new patient facing online portal (available on an internet enabled PC or downloadable App).  My Care gives you greater control of your healthcare by providing you with personalised and secure online access to specific parts of your medical records.

Through My Care you can access information like your medications, appointments and some test results via your digital device.

It is free to use, and it is optional. You can continue interacting with healthcare services via traditional methods – letters, phone calls, etc.

The Northern Trust is live with encompass, the new digital system for patient records. For information to be accessible on My Care it must first be inputted into the encompass system. Members of the public and patients accessing services with any trusts that are live with encompass, are able to access some of their healthcare information including appointments, medications and some test results.

How to log into My Care

  • You can sign up for My Care by downloading the MyChart app from the App Store (Apple/iOS devices) and Google Play (Android devices).
  • Inside the MyChart app you should select My Care from the drop-down menu.
  • By selecting Sign-in through the webpage or the app, you will be redirected to NI Direct.
  • My Care will use your NI Direct account information to prove your identity and provide access to My Care.
  • If you already hold an NI Direct account you can use your current information, if you do not have an NI Direct account you must create one to access My Care.

What information can I see on My Care?

The type of information below is available on the app /portal if you have used services in a trust that is live with the new encompass system.

The South Eastern Trust, Belfast Trust and Northern Trust are now live with encompass.

The following information will be available on My Care:

  • Patient name
  • Patient demographics
  • Patient pharmacies
  • Care Team
  • Active medications
  • Allergies Immunisations
  • Encounters (exclusions apply such as social care)
  • Visits: Upcoming and Past (exclusions apply such as Social Care)
  • Letters (delivered to the patient)
  • Discharge notes/instructions; Notes manually shared to patient
  • Problem list (Diagnoses not shared by default)

Who can use My Care?

You can use My Care if you are:

  • over 18 years old and can make decisions for yourself or
  • under 18 years old and a health and social care professional has agreed you can use My Care.

How do I access My Care?

The My Care patient portal is available as an app or through a web browser on your digital device (mobile phone, tablet or computer). You will need to have an active NI Direct account to access to My Care. For more information on this please visit Identity assurance | nidirect. Can I give someone access to my My Care account?

You can choose to let someone you trust – usually a family member or carer – to access your My Care account to help you manage your care by raising awareness of appointments, or reading and interpreting letters.

This is known as proxy access. You can stop anyone using your My Care at any time and you can decide how much other people can see and what they can do on your My Care record.

How do I get Proxy access?

You may be able to get proxy access if you are supporting someone who is:

  • over 18 years old and cannot make decisions for themselves or
  • a child or young person.

To get proxy access talk to a health and social care professional who will decide:

  • if you can use someone else’s My Care to support them
  • and how much you can see and do on it.

I’ve downloaded My Care – why can’t I see much information?

My Care features will become available as each Trust goes live with encompass. Patients will be encouraged to sign up for My Care following an appointment or hospital stay. Historical information will be limited. The patient record will fill up with information following every interaction with the health and social care system.
