Physical Health and Disability Service
The Physical Health and Disability Service provide specialist services for those with physical disabilities or sensory impairments.
Who are services available to?
The service is available to those who are aged between 18 and 64 years old, who live in the Northern Trust area and have a physical or sensory disability that has a substantial and long lasting effect on their ability to manage daily life.
The Teams can assess your needs to see if you are eligible. They provide access to services at home, day care opportunities, short breaks for you and your carer and/or help to identify options for long term care. They can also provide social work support in the form of advice and counselling to help you cope with the effects of your disability and put you in touch with other professionals and organisations that could offer you support.
We aim to help remove the barriers which stand in the way of people with disabilities pursuing their chosen lifestyle by providing a range of services, support and advice to help them live as independently as possible in their own home.

Sensory Impairment Team
