Connect North: A guide to community services within the Northern Trust area

Many things affect our wellbeing, not just physical health conditions.  Social factors such as work, money, housing problems, the challenges of managing long-term conditions, loneliness or isolation can all have an impact on our wellbeing.

Connecting people with the right help and support is important.

Connect North can help by offering a social prescribing service which connects people to community services. This service is for adults living in the Northern Health and Social Care Trust area.
