Dementia Navigator

Dementia Navigators offer post-diagnostic support for people diagnosed with dementia. This service is provided from 9.00am – 5.00 pm, Monday to Friday excluding Bank Holidays.

Dementia Navigators are social workers, and work alongside Memory Link Workers.

Service users with no named worker who receive a new dementia diagnosis will be offered post diagnostic support.   A support plan will be developed with the service user, carer and family members. Information will be provided on services and supports in the voluntary / community sector.

The aim of the service is to provide support and to reduce emergencies by ensuring service users have a point of contact if difficulties arise.

Referral information

Service users newly diagnosed with dementia who do not have a named worker will be offered this service.

Service users with dementia no longer requiring support of a named worker will also be offered this service.
