Community Mental Health Teams (Older People)
There are five Community Mental Health Teams for Older People (CMHTOP) based in four Northern Trust geographical localities.
The multidisciplinary team provide a range of services to individuals with mental health needs to include, individuals living with a dementia irrespective of age, and individuals with a mental illness aged over 65. In addition the team provides a range of services and supports for carers of individuals known to CMHTOP.
Each team works with a range of professionals within primary and secondary care and the independent, voluntary and community sectors.
The aim of each CMHTOP is to enable each person living with a mental illness and their carer to be supported to lead as independent a life as is possible with support
Referral to CMHOTP
There is a single point of access for referrals through the General Practitioner.
CMHTOP accepts direct referrals from the Rapid Assessment Interface Discharge (RAID) service.
Hours of Service
CMHTOP provides a service from 9.00am – 5.00 pm, Monday to Friday excluding Bank Holidays.
