Memory Service

The Memory Service is a multi-disciplinary service which includes medical, social work, nursing, occupational therapy and administrative support. The service is provided from 9.00am – 5.00 pm, Monday to Friday excluding Bank Holidays.

Assessment

The Memory Service assesses memory difficulties and completes cognitive tests and where required offers a diagnosis of dementia.  Treatment options, support and advice is given at the point of assessment.  The Memory Service will discuss and make onward referrals to appropriate services, for example, for post diagnostic support or assessment for support services.

Support and Memory Rehabilitation

The Memory Service also provide Occupational Therapy Memory Rehabilitation and post diagnostic support from Dementia Navigators.

Referral information

Referrals for assessment of suspected dementia are triaged within the local Community Mental Health Team (Older People).
