My Care patient portal

My Care is a secure patient portal that allows you access to certain parts of your medical record.

The portal is free to use and available through the mobile app ‘My Chart’ and the HSC My Care website.

Who can access My Care?

Any adult with capacity can access their My Care record.

You can also grant another person access to your record.

Anyone under the age of 18 can also request access, if they are assessed by a health and social care professional as competent to do so.

Key features of My Care

  • View upcoming and past appointments
  • Access some test results
  • View shared letters about our care after each visit
  • Access trusted health information resources

Some test results are released automatically and some are released after your clinician has seen and acted on them. You can ask for your specialist to release results for you too.

My Care does not include GP records or results of tests requested by your GP, or historical information stored in older healthcare systems before encompass was introduced.

Social care records are currently not available on My Care, but will be added in the future.

How to log into My Care

  1. You can sign up for My Care by downloading the mobile app ‘MyChart’ from the App store (Apple/iOS devices) and Google Play (Android devices).
  2. Inside the MyChart app you should find and choose the HSC logo from the drop-down menu.
  3. By selecting Sign-in through the webpage or the app, you will be redirected to nidirect.
  4. My Care will use your nidirect account information to prove your identity and provide access to My Care.

If you already hold a nidirect account, you can use your current account information.

If you do not already have a nidirect account, you must create one in order to use My Care.

Once your identity has been verified, you can set up two-factor authentication, such as a pin, fingerprint or face ID, so that you can sign in each time without using nidirect.

When creating a nidirect account, you will need:

  • A valid unique email address not used for anyone else’s account
  • A valid passport or driving license
  • A smart phone/ipad to take a selfie photograph

It can be difficult for younger people of post-primary age to apply for a nidirect account without a passport or driving licence. If you need support, you can contact nidirect.

Proxy access

Any person with a My Care account can grant another person access to their record. This is known as proxy access.

You can grant proxy access to family members or carers to manage your appointments and read letters.
