The Northern Trust Bereavement Pack provides practical information and support for families and carers following the death of a loved one.
The pack is given to bereaved families after a patient dies in a Northern Trust acute or community hospital. It has been developed to ensure families receive timely, compassionate and consistent information to help them understand the next steps and access available support.
The pack includes a range of booklets, leaflets and guidance resources, as well as a letter from the Trust Bereavement Coordinator with information about Cruse Bereavement Support.
Digital versions of the resources included in the pack can also be downloaded or printed if needed.
