Children’s Disability Service

We support children and young people (up to age 18) with complex needs, including learning disabilities, physical disabilities, sensory impairments and serious health conditions.

We have four local teams across the Trust area.

We work with children, young people and their families to help them live as independently and fully as possible.

Assessment of need and support planning
Direct support for children and young people
Joined‑up care with health professionals, schools and education services
Access to local support such as childcare, playgroups and family support groups
Short breaks and other support services
Specialist equipment for children with sensory impairments
Safeguarding support where there are concerns about a child’s safety
Support for children in care where needed

How to access the service

You can be referred by a professional such as a GP, health visitor, or school staff member. If you think your child may need support, speak to someone involved in their care to discuss a referral.
