The Physiotherapy Recovery Service is part of an integrated multidisciplinary team comprising physiotherapy, occupational therapy and social work. The aim of the service is to support a safe and timely discharge from hospital by providing short-term assessment and early rehabilitation intervention, in clients’ own homes or other community settings.
Therapy input may involve working with therapists, therapy assistants, ward-based staff, care staff, and home care providers. There is a strong emphasis on enabling clients to actively participate in, and take ownership of their rehabilitation journey.
Service users may be referred to the service following discharge from hospital, with inclusion dependent on occupational therapy or physiotherapy requirements, and may receive support through community bed-based, home-based, or reablement pathways.

This service is provided Monday to Friday, from 9:00am to 4:30pm.
