The Trust provides a range of Specialist Nursing Services for children with long term conditions. These services include outpatient care across the Trust, structured education programmes and transition support services.
Referrals are accepted from paediatricians and other health care professionals. Services are provided Monday to Friday, 9:00am to 5:00pm exclusive of bank holidays.
The Paediatric Discharge and Transitions Coordinator
The Paediatric Discharge and Transitions Coordinator is responsible for coordinating all discharges and transitions for children with complex health needs which includes the transition of children from hospital to home, home to school and from children’s to adult services. The Paediatric Discharge and Transitions Coordinator works closely and in partnership with the child and family, disability social work teams, the medical teams, acute and community nurses, allied health professionals and other relevant professional staff and voluntary agencies.
