Community nurses (also known as district nurses) provide nursing care and support in your home or at a local clinic.
They look after people with a wide range of health needs, including short‑term illness, long‑term conditions and end‑of‑life care.
Each GP practice has a named district nurse who works as part of your wider care team, alongside GPs, social workers and occupational therapists.

Hospital Diversion Nursing Team
The team provides nursing care at home or in local clinics for people with more complex or urgent healthcare needs, such as:
- Intravenous (IV) treatments
- Blood transfusions
- Palliative care
The service helps:
- Avoid unnecessary hospital admissions
- Support early discharge from hospital
- Reduce trips to emergency departments
