Our Heart Failure Service is delivered by specialist nurses who are highly trained in caring for people with heart failure. They can also prescribe medication to help manage your condition.
This service is for people who have heart failure caused by a weakened heart pump (known as reduced left ventricular function), confirmed by tests such as an echocardiogram or cardiac MRI.

How to access the service
You will usually be referred to this service by a hospital specialist.
Most patients are referred by a consultant or cardiologist after:
- a hospital stay, or
- an outpatient appointment
Referrals can also be accepted from other health trusts for people living within the Northern Trust area
How we support you
At your clinic appointments, the nurse will:
- Assess your condition and symptoms
- Start or adjust your heart failure medication
- Monitor how well your treatment is working
- Check for any side effects
- Give advice on how to manage your condition at home
The nurses work closely with your GP, cardiologist and other healthcare professionals to make sure you receive the best care.

- Support you to manage your condition confidently
- Provide compassionate care, including end-of-life support where needed
When you leave the service
You will be discharged from the service when:
- Your medication has been adjusted to the best level for you, and
- Your condition is stable
