Community children’s nursing services

The Community Children’s Nursing service operates Monday to Friday, 9:00am to 5:00pm exclusive of Bank holidays in 5 teams across 3 locality areas in the Trust.

The Community Children’s Nursing teams provide a service to children who are acutely or chronically ill, have complex health needs and or require palliative and end of life care.

In addition, the service offers support to children with complex health needs following an assessment of need. This can include in home care and supporting care in the school setting.

Referrals are accepted directly from paediatricians and other health care professionals.

  • Children and young people up to age 16 years residing in the NHSCT will have services provided by the Community Children’s Nursing team. This includes referrals from BHSCT. The exception to this is young people aged
  • For young people age 16 to 18 years, local dialogue and agreement is required, between Community Children’s Nursing, District Nursing, Hospital Diversion Nursing team, Community Learning Disability Nursing, as to who is best placed to deliver the service. Shared arrangements and co-working may also be considered.
