Diabetes prevention programmes

This programme is for people whose blood sugar levels are higher than normal, but not high enough to be diabetes.

This is sometimes called pre-diabetes.

You may be offered a place if your blood test shows:

  1. HbA1c: 42–47 mmol/mol, or
  2. Fasting blood glucose: 5.5–6.9 mmol/L

The good news is that you can take steps now to reduce your risk of developing Type 2 diabetes.

What is the programme?

The diabetes prevention programme helps you make small, positive changes to improve your health.

You will get support with:

  • Healthy eating
  • Building better habits
  • Improving your long-term health

What to expect

  • You will attend sessions as part of a small group (up to 20 people)
  • You are welcome to bring a friend or family member for support
  • You will meet a trained coach who will guide you

At your first session, you will:

  • Be asked about your health
  • Have the option to share your weight
  • Receive information and advice to get started

The programme includes:

  • An initial 2-hour session
  • Follow-up sessions lasting 1 hour

How to join

You will need a referral from your GP.

Once we receive your referral, we will contact you directly and invite you to a programme near you
