Dementia navigators are social workers who work alongside memory link workers.

They offer post-diagnostic support for people diagnosed with dementia.

They provide a service which helps to reduce emergencies, by ensuring that service users have a point of contact if difficulties arise.

A support plan is developed with the service user, carer and their family members, and information is provided on services and supports available within the voluntary and community sector.

How to access the service

Service users who are newly diagnosed with dementia, and who do not have a named worker, will be offered this service.

Service users with dementia who no longer require support of a named worker will also be offered this service.

Hours of service

Monday to Friday, 9:00am to 5:00 pm (excluding bank holidays).
