The Northern Trust has five community mental health teams for older people.
The teams provide a range of services to individuals with a mental illness over the age of 65, and to adults of any age with a diagnosis of dementia.
In addition, the teams provide a range of services and support for carers of individuals known to community mental health teams for older people.
The teams are made up of medical, nursing, social work, occupational therapy, support staff and administrative staff.
Each team works with a range of professionals within primary and secondary care and the independent, voluntary and community sectors.
Their aim is to enable each service user and their carer to be supported to lead as independent a life as possible.

How to access the service
The service accepts referrals from GPs, and directly from the Mental Health Liaison Service within our acute hospitals.
Referrals for assessment of suspected dementia are also triaged by the community mental health teams for older people.
