The Memory Service has a team of staff made up of medical, social work, nursing and occupational therapy staff, as well as administrative support.
The service assesses memory difficulties and completes cognitive tests and where required offers a diagnosis of dementia.
Treatment options, support and advice is given at the point of assessment.
The Memory Service will discuss and make onward referrals to appropriate services, for example, for post-diagnostic support or assessment for support services.
The Memory Service also provides occupational therapy memory rehabilitation and post diagnostic support from Dementia navigators.

How to access the service
Referrals for assessment of suspected dementia are triaged within the local community mental health team for older people.
Hours of service
Monday to Friday, 9:00am to 5:00pm (excluding bank holidays).
