The Physical Health and Disability Service provides specialist support for adults with physical or sensory disabilities.

Who can use this service?

You can access this service if you:

• Are aged between 18 and 64
• Live in the Northern Trust area
• Have a physical or sensory disability that has a significant and long-term impact on your daily life

How we can help

Our team will assess your needs to see what support you may be entitled to. We can help by:

• Arranging support at home
• Providing access to day care opportunities
• Offering short breaks for you or your carer
• Helping you explore long-term care options if needed
• Providing advice, counselling, and emotional support
• Putting you in touch with other services and organisations

Our aim is to remove barriers and help you live as independently as possible in your own home.

If you have difficulty with your sight or hearing, our specialist Sensory Support Team may be able to support you.
